CPE Request Registration Cancellation

This form is to request the Cancellation of the Exam Registration.  This doe not provide any refund it only removes a name from the registration screen this opens another slot for another student.

The name and email and date must match the registration before a cancellation will be provided.
Has to be in the future date to remove registration.
